By submitting this form, you (the Customer) authorizes Live Cloud Depositions, LLC (LCD) to charge a $250 non refundable, one-time job set up fee to the credit card listed above, as well as any additional attendee fees that may occur at the conclusion of the event. This contract is non-transferable and non-refundable. The Customer agrees not to hold LCD, its subsidiaries, employees, and agents liable for any claims or damages arising from mistakes, accidents, omissions, interruptions, delays, or error.
